The application period for the Year Twelve | 2025 Edition is now open.
If you wish to apply, it is very important to carefully read the event's REGULATIONS and APPLICATION FORM AND TERMS OF LIABILITY documents.
IMPORTANT INFORMATION ABOUT THE APPLICATION FORM AND DISCLAIMER:
1. Do not distort the form by breaking the PDF format of the file to be filled out by typing. It must be filled out by hand.
2. Paste a recent 3x4 photo in the space provided.
3. Answer all questions in the spaces provided. Include concise and relevant information. Attach additional pages if necessary. Please include photographs, official results, and copies of official finisher certificates for question two on the form.
4. Have your signature and that of two witnesses notarized and register them on the document. Please note: applications will not be accepted without the signatures of the candidate and two witnesses notarized, totaling three notarizations.
5. Send a scanned copy of the completed form (with attachments) to the e-mails aslftriathlon@gmail.com and andrius_slf@hotmail.com. The printed original of the form with the three notarized signatures must be sent to the address UB515 ULTRAMAN BRASIL - Rua Von Martius, 325, sala 505, Jardim Botânico - Rio de Janeiro – RJ - CEP: 22.460-040 – BRAZIL.
6. Along with the scanned copy of the form, send a high-resolution photograph of yourself. If your application is accepted, the photograph, along with your sports resume, will be included in your profile on the competition website.
7. Please wait for contact from the organization.
8. The registration fee is one thousand three hundred and twenty dollars, which must be paid only after the athlete's application has been approved by the organization.
9. For the Year Twelve Edition | 2025, the exchange rate of R$4.69 (four reais and sixty-nine cents) will be used, totaling R$6,197.00 (six thousand one hundred and ninety-seven reais).
